FastWave Medical has received U.S. Food and Drug Administration (FDA) investigational device exemption (IDE) approval to begin the SPARC pivotal trial for its Artero electric intravascular lithotripsy (IVL) system, marking a significant step in the company’s efforts to address peripheral artery disease (PAD).
The approval allows FastWave Medical to move into a late-stage clinical evaluation of Artero as competition within the IVL market continues to intensify. The technology is being developed around a strategy focused on delivering effective vessel treatment while simplifying the procedure for physicians.
Study to Enroll Up to 125 Patients
The SPARC trial will be conducted as a single-arm study involving up to 125 patients across as many as 30 clinical sites in the United States. The trial is expected to generate clinical evidence on the safety and performance of Artero in patients with PAD.
A key component of the program is an intravascular ultrasound (IVUS) sub-study. This portion of the trial is designed to provide detailed imaging evidence of how Artero delivers energy around the vessel, with the goal of documenting its circumferential energy delivery.
Such evidence could help FastWave Medical demonstrate how its system performs within heavily calcified peripheral arteries, where effective treatment can be challenging and where physicians increasingly rely on technologies capable of modifying calcium before other interventions.
Challenging an Established Market
FastWave Medical’s progress comes as several major medical device companies strengthen their positions in the IVL market. Industry leaders including Johnson & Johnson, Boston Scientific, Abbott and Stryker are investing heavily in technologies designed to address vascular calcification and expand treatment options.
For smaller medical technology companies, competing against these established players presents a significant challenge. Large incumbents benefit from extensive clinical networks, established physician relationships and global commercial infrastructure.
FastWave is therefore positioning Artero around potential advantages in speed and simplicity, seeking to differentiate its system in an increasingly crowded field.

The SPARC trial represents an important test of whether FastWave Medical can translate its technology into meaningful clinical and commercial advantages. Positive results could strengthen the company’s case for Artero and provide the foundation for broader regulatory and market ambitions.
