Johnson & Johnson (J&J) has received U.S. Food and Drug Administration (FDA) de novo authorization for its Ottava robotic surgery system, marking a significant milestone in the company’s expansion into the rapidly evolving robotic surgery market. The approval, announced ahead of a major robotic surgery conference and earlier than many industry analysts had anticipated, has generated considerable attention across the healthcare sector.

The FDA authorization covers multiple general abdominal surgical procedures, positioning Ottava as a new competitor in a market long dominated by Intuitive’s da Vinci system and increasingly challenged by Medtronic’s Hugo platform.

A Different Approach to Surgical Robotics

Unlike conventional robotic surgery systems that rely on large carts and external boom-mounted robotic arms, Ottava introduces a fundamentally different design. According to Rocco De Bernardis, Global President for Ottava, the system integrates four intelligent robotic arms directly into a standard-sized operating table.

This integrated architecture allows the robotic arms to remain hidden beneath the table when not in use, reducing clutter in the operating room while giving surgical teams greater flexibility during procedures. The design also enables the operating environment to resemble a conventional surgical setup, making it easier for healthcare providers to adapt to robotic-assisted workflows.

De Bernardis explained that the engineering challenge was not simply to develop another robotic platform, but to simplify the surgical experience by removing unnecessary complexity from the operating room.

Leveraging J&J’s Surgical Expertise

Johnson & Johnson believes its decades of experience in surgical instrumentation and tissue management provide a strong foundation for Ottava. The company says the platform has been developed to improve precision while helping reduce patient complications through advanced surgical technologies and refined tissue-handling capabilities.

The robotic system is also integrated with Polyphonic, J&J’s digital surgery ecosystem. The platform is designed to support surgical learning, data-driven insights, and future innovation by serving as an open and secure digital environment for connected healthcare technologies.

By combining robotics with digital intelligence, the company aims to enhance surgeon training, improve procedural consistency, and create opportunities for continuous advancements in minimally invasive surgery.

Entering a Competitive Market

Robotic-assisted surgery has experienced rapid growth over the past two decades, with Intuitive establishing the category nearly 25 years ago. Since then, several major medical technology companies have entered the field, making competition increasingly intense.

J&J believes Ottava’s differentiated design and integration of robotics, digital technologies, and surgical expertise will help the platform stand out as hospitals continue investing in next-generation operating rooms.

Speaking about the engineering behind the system, De Bernardis said developing Ottava required bringing together some of the industry’s best engineering talent to solve a highly complex technical challenge. The objective, he noted, was to eliminate bulky robotic structures and instead create a solution that gives surgeons greater simplicity and flexibility without compromising performance.

With regulatory clearance now secured, Johnson & Johnson is expected to begin introducing Ottava to healthcare providers, marking a major step in its strategy to expand its presence in the global robotic surgery market while offering hospitals an alternative to existing robotic-assisted surgical platforms.|
